Colina Academic Petition Form
Students must petition for an honors class if they are currently in a CP class and would like to move to Honors in the subject, or they have received a grade of C+ or lower either semester in an honors class. An Academic Petition is a formal request for reconsideration of a course placement. Your request will be considered by an Academic Petition Committee. Placement decisions are based upon student grades, test scores, information provided in the petition, and space availability. Students will be notified of the petition decision on the schedule they receive during Orientation. *Priority Deadline for Petitions is March 28th, 2024.*

*You must petition for each class you are requesting (ie: two classes require two separate petitions). All petitions will be carefully reviewed to determine placement decisions.*

Your signature will also serve as agreement to the following academic success contract: I agree to maintain my academic grade in this class. This contract will be assessed at Progress Reports and at Semester Report Card periods. I understand that I need to maintain an 80% at every marking period in order to continue in my current academic placement. I understand that absences/tardies will negatively affect my grade. I agree to complete and submit my homework/classwork assignments on time, study for my quizzes and tests, communicate with my teacher(s) when I don't understand a concept or assignment, record all of my assignments in my assignment notebook, earn passing grades in all of my other classes, and maintain consistent and timely attendance. I understand that failure to follow through on any of these items may negatively influence my academic grade and I can be dropped from the class.
