NOTE: The tournament is very close to maximum capacity for open events. If you wish to enter an open event please email me (healesvilletennisclubopen@gmail.com), and I'll see if can fit you in. Under 17s entries are still being accepted.

If you're entering a doubles event, please give the name of your playing partner. If you're entering multiple doubles events, please give the names of your partners on separate lines, also specifying the event. If you wish to enter a doubles event but don't have a partner, write "partner required" and we'll try to find someone for you.
