There are FOUR portions to the statement below. All 4 of them must be true for you to qualify.
1. I am at least 18 years old.
2. My child/student is a part of San Diego Unified School District or Poway Unified School District
3. My child is currently Distance Learning, or has been a part of Distance Learning.
4. My child was between ages 5-10 during Distance Learning.
