Regional Dance Permission Slip SOAR

SOAR is sponsoring a REGIONAL DANCE at Salisbury Central School on Friday, May 31st from 6:00-8:30 P.M. It is open to all 7th and 8th graders in Region One. The theme is BLACK and WHITE so wear anything (school appropriate) in those color schemes. 

In order to attend, your child must be on the guest list by filling out this permission slip no later than Tuesday, May 28th. No walk-ins will be allowed. Your child must be on the guest list to attend. This permission slip and an admission fee of $10.00 will be required at the door. Cash or Venmo (@soarkids) will be accepted. *If you filled out a permission slip for the previous date, you will need to fill out a new one.*

The $10 fee includes one snack and drink of choice, as well as one raffle ticket to be entered to win prizes.

Other snacks, drinks, pizza, and raffle tickets will be available for purchase. Please indicate on this form if your child will be interested in purchasing pizza.

Please be at the school promptly at 8:30 P.M. to pick up your child.  If they are going home with another student, please note that information below.  If they are allowed to leave the dance earlier than 8:30 P.M, please indicate below who will be picking them up and at what time. Once a student leaves the dance, they will not be allowed to return.

My child and I have BOTH read and understand the rules for this event: RULES FOR ALL REGIONAL DANCES: 
1. You must have completed this form to have your name listed on the roster. 
2. No student will be allowed to leave the dance early without parental permission. A student who leaves early will not be allowed to return. 
3. No cell phones, book bags/backpacks or coats will be allowed in the dance. They will be stored in a separate area if brought to the dance. 
4. Only students dressed appropriately, following the school dress code, will be allowed into the dance. Students will not be allowed to change clothing at the dance. 
5. Students exhibiting inappropriate behavior/bodily contact will be sent home. A parent or guardian will be called. 
6. The home school will be notified of any inappropriate student behavior of and that student may be subject to disciplinary actions.
