This application form will be used to gather details of applicants, in order to issue an Annual FOBS Perks Card by a member of the FOBS team.

Friends of Brook Field School (FOBS) have been working hard over the last several weeks to get this great initiative up and running, where we are giving back to parents, carers, family or card holders, whilst supporting local and parent businesses and of course helping raise funds for the school...a win-win-win situation, especially given the cost-of-living crisis.
How it works?
In simple terms the initiative involves a donation being made by a person to obtain the FOBS Perks Card annually, where the holder can then get discounts/perks with certain businesses or organisations.
How to apply?
To apply for the period 01/09/2022 to 31/08/2023, please fill in this form.
Once you have applied we will contact you to arrange a time to collect your card and details of how to make your donation.
What businesses are included?
The business directory, detailing the businesses/organisations that have kindly agreed to join this initiative, will be held and maintained weekly at https://www.brookfieldprimary.org/friends-of-brook-field-school-fobs-upcoming-events/

We know the coming months will be tough for a lot of families, and we are hoping this initiative will help families save money whilst supporting local businesses.
The 'Fine Print'
(i) FOBS will maintain a list of businesses or organisations that are willing to support discounts/perks for card holders.
This list may expand, may decrease or perks offered may alter during the academic year at the discretion of the business or organisation.
(ii) The FOBS Perks card must be mentioned at the point of booking, ordering and payment. Photo ID of the named holder should be provided alongside the presentation of the card, if requested by the organisation or business.
(iii) The FOBS Perks Card is for the use of the named holder only and must not be shared.
(iv) Misuse of the FOBS Perks Card will result in the card being returned to FOBS.
