Recital Program Ad Submission Form

Guidelines for placement of an ad in our Recital Program are as follows:
1. EMAIL pictures & pre-designed ads to ads4program@gmail.com after completing this form.   Please reference the dancer’s name in the subject line and note the size of ad purchased within the email.  Hard copies will be accepted at the studio, but email is preferred.

2. Pre-designed Ads must be submitted as a high resolution .jpg or .pdf file.

3. All ads must be received between now and MARCH 31 in order to be included in the program.  Late submissions will not be accepted. Please do not wait until the last minute. We can be more creative and do a much better job of proof reading when we have adequate time to process your ads.

4. If your ad is to be designed by our staff, please submit your desired text below on this form and any photo(s) via email.  Photos must be a high resolution .jpg or .png file attachment.

5. Payment can be made at the studio or online at www.illuminationdance.com/shop. Cash or check preferred. Credit card payments will be charged an additional 3% processing fee.
