My son/daughter has my consent to participate fully in the activities of the school boat club and the information given above is, to the best of my knowledge, accurate.

I give consent for my child to be photographed for the purposes of Boat Club publicity and to be videoed for training purposes.

I give consent for my child to participate in club activities both in Monmouth and outside Monmouth. Travel to these events is by minibus/coach/private hire/private car (car only by specific prior notice and this is typically for a few athletes attending Great Britain/Wales trials). Minibuses/coaches are provided by the school from an approved commercial hire firm. Rowing activities are covered by insurance purchased as above with the racing licence and by a 3rd party insurance policy held by the club/school. In addition, the school holds a specific “off-site” activities policy.

Off-site activities travel and personal injury insurance is provided by Royal Sun Alliance. For summary of cover, please visit the School’s website at www.monmouthcomprehensive.org.uk under Trips. I agree to adhere to the payment requirements.

My child must bring their relevant medication/equipment, including inhalers, to all training sessions and rowing events.

I understand that although Monmouthshire County Council’s guidelines indicate that it is preferable for adults of both genders to accompany day trips with both boys and girls, this may not be possible. I understand that I have the right to withdraw my child on such occasions. Should I have such a concern I will make it known at this time. Overnight trips require same gender adult coverage and, as such, we always comply. I further understand that due to limitations on staffing, many of the adults who accompany trips are not members of school staff, being typically parents or former students. All of these people are approved by the school and hold DBS checks.

I understand that the school’s photo of my child with be passed to British Rowing for printing on their British Rowing Racing License.

This form gives parental consent for ALL rowing club events in the coming academic year.
