Please fill out this form to request to be a COVID-19 Emergency Food Distribution site distributing Farm to Families fresh food boxes through the Share Food Program.

All potential distribution sites must:

• Have the capacity to receive and distribute at least 3 pallets(~150 boxes) of food per delivery
• Follow all requirements around social distancing at distribution sites
•      Have enough staff/volunteers to manage distribution
