We are a group of researchers from You're Wonderful Project;, an organisation working towards mental health awareness and accessibility, and we are conducting a research assessing the impact of media upon children of a particular age group. The questions in this google form are asked in order to help us understand the different types of media children are exposed to and prefer to watch.

IMP: If you are a child aged 15-18 years, kindly ensure to seek parental consent before filling in this form. If you are a parent filling in this form for your child, please note that the questions need to be answered according to the child's preferences for media.

It should be noted that all information you choose to divulge in this form will be kept confidential and will only be used for research purposes. Your consent will be taken before being asked to proceed to the form.
