Thank you for your interest in becoming a mentor in the Fayette County Schools. Mentoring is about "one child at a time". The mentoring impact has made a difference in the social and academic life of a student, academic achievement, and has a proven record to improve school attendance and behavior. All mentors go through a screening process and training before being matched with a student.

 VOLUNTEER RELEASE STATEMENT: I, the undersigned, hereby state that if accepted as a Friends Mentoring volunteer I agree to abide by the rules and regulations of the program. Further, I understand that I must attend a mandatory orientation and other training sessions as advised. I understand that the program involves spending a minimum of one hour each week at school with my mentee from September through May. I can continue in the program as long as the school and I agree. I understand that the Friends Mentoring Program and the relationships established take place during the confines of the school day in Fayette County Schools. It is not part of any relationship established between mentor/mentee and family members beyond the school day. Any deviation from this guideline is not part of the program and will require clearance from the school coordinator and permission of the parent/guardian. I agree to maintain the identity of the student that I mentor in strict confidence, as well as any information received to guide my assistance to the student. I understand that any information that indicates a child might be at risk should never be withheld, but should be reported to the teacher and/or school administrators immediately. I have not been convicted of or pled guilty or admitted liability to, within the past ten years, any felony or any misdemeanor involving an offense against a person or family, of public indecency or any other sexually related offense, or any violation involving a state or federally controlled substance. I am not under any current indictment, criminal charge or investigation. I understand that a nationwide criminal background check will be made to verify the above. I have read the above Release Statement and agree to its contents. To the best of my knowledge and belief, all statements in this profile application are true and accurate. *
