Creating Well-Being in Schools:                                Initiatives from NY State Schools & Mental Health Organizations
Thank you for your interest in contributing your ideas and initiatives, which will be shared along with other NY State school districts and organization initiatives to help foster a sense of well-being in our schools and communities.  The Creating Well-Being Sharing Site has established a wide audience of viewers who are educators and Mental Health clinicians throughout the US and abroad. We look forward to ongoing conversations about student and staff mental wellness and an opportunity to share ideas in support of one another.   We currently welcome submissions from all NY State Schools and Mental Health Organizations that have a working relationship with NY schools. 

Please complete this form to begin the process of submitting your initiative to the Creating Well-Being Sharing Site in order to provide us with some preliminary information, including the name and contact information of a Liaison with whom we will communicate. Your initiative will be discussed with a member of the Creating Well-Being Sharing Site Planning Committee and once approved, will be posted on LI SEL Forum Website which is collaborating with NY Schools to host the site.

Your initiative will be viewed via a link that you provide to your organization's website, where your document/video/article, etc. is housed, or through a PDF that you provide. We recommend that the document include a copyright or permissions statement, advising the viewer about any third party use of your document.
