The Cosplay Contest will begin at 2:00 pm on the Second Level at the Desk.

I agree that I have read and will comply with the rules of the Fountain City Fandemonium Cosplay Contest, in partnership with the Williams County Public Library (WCPL) and I further consent to the use of my picture, video footage, and the likeness of my cosplay by Fountain City Fandemonium and Williams County Public Library in future promotional efforts. See Below.

I understand that my personal information will be stored with WCPL only and will not be shared with any outside party other than Fountain City Fandemonium.

I understand that I MUST report to the Second Floor Desk for the Cosplay Contest at 2:00pm to complete the entry process; if I do not appear, I will be disqualified from the contest.
