Please fill out to receive further information about specific athletic programs at Godinez Fundamental High School.  Answer all questions and submit this form so the Coach has your information.  

All students interested in trying out for sports must be cleared prior to the determined tryout date.  Go to www.athleticclearance.com to register online for sports and get cleared.  You may only be involved in one sport per season.  YOU MAY REGISTER ONLINE AFTER JUNE 1, 2021.

You need to turn in the clearance confirmation sheet and a copy of your current physical to the Godinez Athletic Director - Mr. Snyder prior to participation.   Information can also be found at www.godinezathletics.com
