Dear Families,
In accordance with Chancellor’s Regulation A-801,  the DOE provides transportation to all ELIGIBLE New York City students in public schools. If you are interested on your child having transportation services, please complete this form as soon as possible.  

*I understand that completing this form does not guarantee a spot on the school bus*
Transportation eligibility is based on a student’s grade level, walking distance between home and school, and existing accommodations based on a medical condition, housing status, or safety assessment. Please use the below graphic for grade and distance requirements eligibility.
