We are so excited to welcome you to RYLA! Before we see you in May, this form is to be completed by students along with their parents/guardians. Before filling out this form, students/parents should receive acknowledgement from their local Rotary Club and/or School District that they are being sponsored by that club to attend RYLA.
You will use this form to submit the following:
- Your contact and personal information (Both student and parent/guardian)
- Confirmation of your sponsoring Rotary Club
- Acknowledgement of the Emergency Contact & Consent Form
- Completion of the Health Form (be prepared to include any medications, allergies, nutritional requirements, other medical limitations/accommodations)
- Acknowledgement of the Code of Conduct
