This form allows parents/guardians to provide feedback about individual volunteer coaches in the West Side Soccer program.  We greatly appreciate this feedback because it helps us to improve the training and management of our volunteers.  We limit the information you provide, and your identity, to WSSL senior leadership.  Your comments and identity will not be given to the coach and will be kept confidential.
