I hereby request the rental/use of the clubhouse facilities of Green Acres Country Club for the purpose of a private party or other private function. I understand that my party will not be considered without this form being properly completed and the necessary payment remitted to Green Acres Country Club, Inc.. I certify that I will personally attend this function and that I will be personally responsible for the actions of my guests and myself while on the premises of Green Acres Country Club. I agree that I shall indemnify, defend and hold harmless Green Acres Country Club, its employees, agents or representatives for any claims arising out of such rental/use of the facilities or equipment of Green Acres Country Club. I also understand that the below party should not be considered approved until I have received verification from a board member.

You will be responsible for a $475.00 cancellation fee if you cancel your event less than 14 days prior to your reservation date. $475.00 deposit due to secure booking date and the remainder is due 30 days prior to event
date(full amount and deposit due at time of booking if less than 14 days before event date)

There will be no clubhouse, pool, or ground rentals to non-members from Memorial Day until Labor Day each year.
