** All refund requests for shows in November must be submitted no later than Monday, October 25th **

PLEASE NOTE:
- This form is to REQUEST a refund. You will be notified within 72 business hours of UA receiving this request as to what to expect next.
- We kindly ask that you please consider holding onto, selling, or giving away your tickets to someone who will use them. Music Venues were the first to close and last to reopen and every bit of support helps ensure we can continue to keep the doors open at Underground Arts. Thank you for your consideration.
