Please complete this form to complete the registration for the Intro to SWI class.  Once payment is received, your name will be added to the class roster and you will be emailed a link to the Zoom room. If you have not paid yet, there is a link at the end of this form. If you have already paid, thank you.

A Zoom link will be emailed to the address you provided at the top of this form the week prior to the class starting. It will be the same link for all class dates so please save it in you calendar or browser. If you need to contact me prior to class, please email at EmpowerLC.lisa@gmail.com. 

Refunds will be permitted if cancelled 1 full week in advance. If participant cancels 3-6 days in advance of the class, then 50% of the class fee will be refunded, no refunds if cancelled within 48 hours of the class. Refunds after class begins are not permitted. There is also an option to apply your payment to a future offering of the class or to reschedule with the presenter.
