This form is intended to collect information for the shared membership directory. If you have made a donation this year of $40 or more, you are a member in good standing and are eligible to be included in the directory. If you're already listed in it please simply include any changes or indicate no changes are needed. Please include only the information you wish to be shared on the membership roster (For example, leave the "email" section blank if you do not want to share your email.)
