Consent
Please complete this form, it will be taken as consent to play. 
Year 9-13 volleyball players must be available for their midweek games (Tues or Thurs) and transported to Tauranga (carpooling is encouraged).
Senior A teams also play in the BOP competition on 3 weekends and NZSS national tournament (17th-23rd March). Other senior teams are also welcome to play BOP.
Year 7 & 8 volleyball is held at KKC in the Action Centre after school

Uniform
Year 7-10 play in KKC PE uniform and indoor shoes. Kneepads are recommended.
Year 11-13 will have uniform shirts distributed and need own black shorts, shoes & kneepads (recommended).

I understand that there are risks associated with involvement in school sports events and that these risks cannot be completely eliminated. I understand that the school will identify any foreseeable risks or hazards and implement correct management procedures to eliminate or minimise these. I will do my best to ensure that my child understands they must follow these procedures. I know that I am able to ask any questions of the school about the activities my child will be involved in, to gain a better understanding of the risks involved. I recognise that participation in such activities is voluntary and not mandatory through a ‘challenge by choice’ procedure (challenge by choice’ means the participant chooses their own level of challenge within a supportive peer environment). My child and I both understand that they may withdraw from an activity if they feel at risk. This must be done in consultation with the person in charge. I understand that the school does not accept responsibility for loss or damage to personal property (either my child’s property or damage to other’s property caused by my child) and that it is my responsibility to check my own insurance policy. I also agree to meet expenses of getting my child home should it be necessary for them to return early from the trip.

I agree that IF prescribed medication needs to be administered, a designated adult will be assigned to do this. I will ensure that prescribed medication is clearly labelled, securely fastened and handed to the designated adult with instructions on its administration.
