The purpose of this form is to determine whether your student will be attending school as part of our hybrid model in-person or only virtually. Virtual learning will look different than it did in the Spring, and will include up to 50% synchronous learning (this means that students at home will be taught live at the same time as students in the classroom). The schedule for synchronous learning will take place within the in-person school hours.  

Please submit one form per student.

Please note that schools require 5 school days of notice should you change your mind. Please notify your student's school via phone if this is the case.

El propósito de este formulario es determinar si su estudiante asistirá a la escuela como parte de nuestro modelo híbrido en persona o sólo virtualmente. El aprendizaje virtual será diferente del de la primavera e incluirá hasta un 50% de aprendizaje sincrónico (esto significa que los estudiantes en casa recibirán clases en vivo al mismo tiempo que los estudiantes en el salón de clase).
