As a member of CAUK we really want to know how well our work supports you.
We would be very grateful if you could answer as many questions in this survey as you can. It should take no longer than 20 minutes.  Although there are 30 pages in the complete survey, many will be skipped if they are not relevant to you from the responses you give.

The information you give in this survey fulfils several important functions for CAUK:
• It will provide essential information for grant proposals:
       o from what you say about the different types of support we provide
       o from the questions about your experiences, we can tell funders what life is like with cavernoma
• It will help us shape the support that CAUK gives in the future.

We would like views and thoughts from all of our members. Parents can complete this on behalf of their children although we encourage young people with cavernomas to complete the survey themselves, if they can.

We would really appreciate any members with cavernoma, who also have a child/children with cavernoma, to complete the survey twice. Once for themselves and once on behalf of their children.
