In order for us to plan appropriately, please fill out this short form so we can anticipate the number of children planning to test on Tuesday. THIS IS AN OPTIONAL SERVICE BEING PROVIDED.  Students should be dropped off as usual and go to their classroom. Please complete this form by Monday at noon if you are opting to have your child tested. This is not mandatory, but appreciated for organizing the morning. If your child is being tested, please be sure to send them in with the requested authorization form as well as a copy of your driver's license and insurance card if you have one.
