Please fill out this survey if you are you are choosing to have your child receive the Pfizer vaccine at LACS on either Thursday, May 20th or Tuesday, May 25th. Filling this out will help us to get an idea of what kind of supplies and support Public Health will need to provide for us. Those who are not receiving the vaccine do not need to fill out the survey. Please still send in your registration form that was sent home in the mail. Each student should have a separate survey.
