本人了解菁英中文學校 在教學期間，為提升學校教學品質、促進學生學習意願或記錄活動花絮，將不定期使用在校內、外拍攝其中可能包括我本人以及孩子的照片。 

本人在此同意充分授權菁英中文學校使用本人及孩子的照片或作品(含美勞、寫作和影音)，這授權使用範圍包括：學校網站、Google Classroom、海內外華語教學報告、海內外從事華語教學工作的教職人員經驗分享、媒體和僑委會等和教學相關的活動。

本人亦了解菁英中文學校於以上推廣正體中文教學相關的非營利活動中，將可能使用、複製或展示包含本人及孩子的照片或作品。

個人隱私或安全考慮：

若本人及孩子的安全或隱私因此授權而受到影響，本人同意做法如下：應該由我本人主動以書面方式通知校方，並請校方停止拍攝照片。如有必要，應由本人主動向校方指出欲取消授權的照片或作品。

備註：本同意書僅於學生入學時填寫一次


I acknowledge that at Elite Mandarin School, during the teaching period, photos may be taken on and off the school premises for the purpose of enhancing the school's teaching quality, promoting students' willingness to learn, or documenting activity highlights. This may include photos of both myself and my child.

I hereby grant full authorization to Elite Mandarin School to use photos or works (including art, writing, and audiovisual) of both myself and my child. The authorized usage includes, but is not limited to, the school's website, Google Classroom, Mandarin teaching reports domestically and internationally, experiences shared by Mandarin teaching professionals domestically and internationally, media, and events related to teaching organized by the Overseas Community Affairs Council (OCAC).

I also understand that Elite Mandarin School, in non-profit activities promoting Traditional Chinese teaching, may use, reproduce, or display photos or works that include both myself and my child.

Privacy or Safety Considerations:

If the safety or privacy of myself and my child is compromised due to this authorization, I agree to the following actions: I shall actively inform the school in writing and request the school to cease taking photos. If necessary, I shall proactively indicate to the school which photos or works I wish to revoke authorization for.

Note: This consent form is to be filled out only once upon the student's enrollment.
