The Nashoba Youth Hockey Association (NYHA) offers Tuition Assistance to members who request and demonstrate the need. Please use this form to request Tuition Assistance.

How to Make a Request
Before accepting placement and making the first payment:

1. Read the NYHA Tuition Policy
2. Complete and submit the NYHA Tuition Assistance Request Form (this form)
3. Submit supporting documentation to the NYHA Treasurer (if requested)

Confidentiality
NYHA takes measures to keep your information confidential and requests documentation only when necessary. By submitting this request, you acknowledge that the NYHA Executive Board will review the application and any supporting documentation you submit.

Notification
The NYHA Board will notify applicants of the decision in writing. Players and parent(s)/guardian(s) must agree to follow all NYHA policies and maintain membership in good standing to continue receiving financial support.

Terms

• Assistance is granted on a per season basis.
• Applicants must apply each season.  Applicants must remain members in good standing throughout the season.
• The amount of assistance varies and is dependent on the availability of funds and specific needs of the applicant.
• Assistance is awarded at the full discretion of the NYHA Executive Board of Directors.
• Tuition assistance is offered as a credit toward the player’s tuition balance and will not be provided as cash.
• Tuition assistance will be applied to the next season tuition and may not be applied to current outstanding balances.
• Failure to pay balances according to the agreement will result in the player not being allowed to participate in NYHA activities (including but not limited to skills, practices and games) until outstanding balances have been paid in full.
• All decisions made by the NYHA Board are final.

By completing this form, you acknowledge that you have read, understand, and agree to the NYHA Tuition Policy and that NYHA cannot guarantee your request for assistance will be awarded.
