Please submit this form if your child is unable to attend school. For High School and Middle School reporting, please fill out by 8:00 am or you will get a phone call. You may make a submission in advance if you know your child will not be in attendance on a given day or days. Please submit a separate form for each child. You will receive an email from a building secretary to confirm they have gotten your response. Please also be prepared for follow up questions in the event more information is needed.
