Johnson City Schools Hall of Fame Nomination Application
Purpose
The purpose of the Johnson City Schools Hall of Fame is to recognize and honor those educators, alumni and contributors who made significant impacts in progressing Johnson City Schools.  In doing this, we hope to preserve the rich history of Johnson City Schools and establish a link to the past and present members of Johnson City Schools.

Nomination Instructions

1.  You may use this form to complete the application and submit your letter of recommendation (minimum 250 words).  The person submitting an application on behalf of a nominee is fully responsible for making sure all information is complete and accurate.

2.  Submit a one page, double-spaced biography highlighting the nominee's contributions to Johnson City Schools by email to Collin Brooks at brooksb@jcschools.org.    

3.  Submit two (2) additional supporting letters of recommendation (minimum 250 words each) explaining why the individuals are in support of the nomination by email to Collin Brooks at brooksb@jcschools.org.
