Parents or legal guardians who wish to transport their student-athlete to and/or from an athletic event must submit this form for each date in which an alternate method to the team transport will be used.  Students will not be allowed to transport themselves to and/or from events.

In doing so, I am voluntarily waiving any and all claims and release Fort Worth ISD and all FWISD & Benbrook Middle-High School employees, board members and representatives from any and all liability, damages, fees, or costs arising from or in connection with an alternative method of transportation to and/or from the school trip. *
