Covid-19 Supplement to Student & Family Handbook
This COVID-19 Supplement to the GALA Student and Family Handbook supplements the terms of the GALA Family and School Contract for the 2020-2021 school year.

Parents/Guardians acknowledge and agree:

1. Prior to sending your child to school each day, you will screen your child for symptoms of COVID-19, which currently include:
o Fever or chills
o Cough
o Shortness of breath or difficulty breathing
o Fatigue
o Muscle or body aches
o Headache
o New loss of taste or smell
o Sore throat
o Congestion or runny nose
o Nausea or vomiting
o Diarrhea

2. You will NOT send your child to school in-person if they tested positive for or are showing any COVID-19 symptoms.

3. Notify the school of your child’s testing positive for, diagnosis of, or exposure to COVID-19. Contact for reporting: EDavis@GALACleveland.org, (216) 413-6244

4. Students will abide by all safety and sanitization protocols as directed by the school, including, but not limited to, social distancing, face coverings (unless explicitly exempted), hand washing, sanitizing, quarantining, and isolation.

5. The school is required to report positive COVID-19 cases amongst students and staff to local health department/s.  Because contact tracing is the best way to identify those who may be at risk for exposure, you agree to cooperate in the health department’s and school’s inquiry.  

6. Cooperate with the school in safety screenings, such as taking temperatures upon entering the school building.

7. Cooperate with the school in an appropriate plan for your child’s absence if needed, including quarantining and isolation, and a plan for their safe return to school in person.
 
8. Pick-up your child from school within one hour of being notified that they are sick or identified as experiencing symptoms.
