Registration form for our service this Sunday, April 3rd. Our service will begin at 10 AM.

Our weekly in-person Sunday services have resumed! Pre-registration is required to attend our service due to government regulations and limited spots. Remaining spots will be for walk-in attendees.

*You can register as a weekly attendee (i.e. you do not have to register again for the following weeks) or register only for this week's service. This form can only register one person at a time, so please register other family members separately. This form will close once our capacity for the service is full.*

Please read the health protocols sent to you in your confirmation email before our service.

We'll send an email by Saturday if there are any changes due to government updates or other reasons.

Please submit this form as soon as possible to ensure your spot and help us plan ahead.

Please only register if the below statements apply to you and your family:
- I have completed Ontario's self-assessment: https://covid-19.ontario.ca/self-assessment/
- I have read FRP's health protocols document and agree to follow the protocols
- I have not traveled outside of Canada in the last 14 days
- I am not sick or displaying symptoms of COVID-19: fever, breathing difficulties, breathlessness, cough, sore throat, fatigue or tiredness
- If I have been in contact with anyone infected with COVID-19 in the last 14 days, I have been tested and my result was negative and I continue to self-monitor for symptoms (if you have not received your results yet, please abstain from attending)
- I will contact Brigette (brigettesirineo@gmail.com) immediately if my situation changes and I am no longer able to attend under COVID-19 regulations.
