CPCC Youth Parents,

The youth are planning Corn Maze excursion on Friday, October 13. The plan is to gather at the church at 6:10 and to be out at the Strawberry Ranch to start the maze at 6:30. There will be adult drivers in place (licensed and insured) to drive the group out to the corn maze and back to the church for 9pm.

Please indicate below if you give permission for your youth to go on this field trip. All will be done to keep your child safe but there still remains a risk of injury. In completing this from you acknowledge this risk.
