Program Contact and Information Survey
Thank you for partnering with the School District of Philadelphia (SDP) for your students’
student teaching experiences.  This form is specifically for a partnership agreement for the purposes of student teacher placements (Stages 1-4).  In order to receive an execution-ready MOU, please complete the form below.

You will be asked to include the contact information for the three key staff members within your institution: MOU authorized signer, programmatic point of contact and placement coordinators. You will also find several questions pertaining to the scope of your student teaching program. If you do not know some of the legal pieces of information, please ask for assistance from someone at your institution such as your General Counsel or Chief Financial Officer.
