Health Form Reminder
All participants are required to turn in a COPY of their BSA Health Form Parts A&B on the first day of camp. Due to HIPAA laws and BSA policies, DO NOT EMAIL your health form. For a fillable PDF of the health form, go to www.scouting.org/filestore/HealthSafety/pdf/680-001_AB.pdf. Please include a copy of the front and back of your insurance card.
