Use this form to cancel your registration and request a refund. Please view our refund policy below. FOSC has fees taken out of each transaction that we cannot get back. We also pay non refundable membership fees in July & August.

*FOSC has transaction and league memberships fees for each player. These are non-refundable to our club.

FOSC 2023 refund policy

U6-U19 Players*:

  • April 1 - June 30: Refund of registration fee, less $15 admin fee per registration canceled
  • July 01 -July 31: Refund of registration fee, less a $30 admin fee per registration canceled
  • Aug 01-Sept 15: Refund of registration fee, less a $35 admin fee per registration canceled 
  • No refunds after Sept 15. 
* If you  originally registered two players and took advantage of our sibling discount during your original registration transaction, you will lose this discount. The sibling discount will be deduted from your refund. If your registered 3+ players, you will be able to keep the additional sibling discounts. 

No refunds after September 30th.
