Morgan State University Event Contact Tracing Form
According to the Baltimore City Health Department, colleges and universities must provide a sign in or ticketing mechanism that tracks guests in attendance for contact tracing purposes.

Instructions: Please fill out one form for each guest attending commencement ceremonies.

*All students and guest are required to complete this form on or before each event to ensure an accurate account of each attendee.
