Please indicate on this form what disability access you are requesting. You must fill out a SEPARATE FORM for each guest requiring accessibility.

With the exception of the accessible seating section and the sign language interpretation section (booked only by request), we are unable to reserve specific seating for guests at Ivy Day or Commencement.

You will receive a confirmation email after your request is approved that your guest has been added to the accessible seating section.

Each guest requiring special seating may be accompanied by one family member in this section due to limited space.
