For you to be eligible for the prizes available in the IHF Youngsters' Training Diary contest, we must have your consent. Please be sure to complete this form along with your video/photo submissions for the IHF Youngsters' Training Diary contest.

For individual entrants, the consent form should be completed by a parent/guardian.

For team entrants, a submission from the team coach or a parent/guardian on behalf of all the parents/guardians of children in the videos or photos is required. Parents/guardians should give their consent to the coach or representative parent/guardian for their child's participation.
