This form serves two purposes:
1. To update the church's contact information for each Member or Friend of CAC.
2. To generate a digital Church Directory to allow members and regulars to contact one another directly.

The responses below will include fields to indicate when you want information shared in the published Church Directory. All responses will be assumed to be shared only with the church unless this field is checked.
