Missed Class & Catch Up Class
This Form must be completed no later than 24 hours AFTER your child's class in order for your child to receive a catch up class within the same term. This form MUST be completed at least 24 hours BEFORE your child's Catch Up class.

Catch up classes are only available within the same term.  

Should you be unsure of the catch up date please leave this blank. When you are ready to arrange a catch up class, please complete this form again.

A Star Dance will only notify you if there is a problem with your catch up class. If you don't hear from us please take your child to the catch up class you have requested.
