What is your experience level with horses (beginner, intermediate, advanced)? Please describe in detail, include if you have taken riding lessons and for how long. 

*If you are not cantering/loping comfortably you are a Beginner. Overstating your riding level could lead to injury.*

Descibe the activities you plan with the horse, ridden or otherwise. *
Who will ride the horse? What is their level of experience? *
Where will you keep the horse (boarded, home, other)? Please provide the name, address and contact information for the facility or property owner. *
Describe stalling and pasture or turnout area (size), fencing, shelter type, population of horses sharing the turnout and the stalling/turnout routine you plan (eg. in 12, out 12, full outside care, etc) *
How often will the horse be fed? What grain and hay will be fed? Does the horse have access to constant clean water? *
How often will you vaccinate the horse? What vaccines will be administered? *
Please describe your worming program *
Who is your CURRENT equine veterinarian? (Include contact #) *
Who is your current farrier? (Include contact #) *
Please provide two references (other than family members) who can verify your ability to provide proper care for the horse. Include email and phone number for each. *
If you have never owned a horse and are NOT planning to board the animal in question your application may be denied. Horses require a large financial and time commitment, and this responsibility should not be taken lightly. *
I, the adopter, understand that there will be a separate Care Contract covering specific care terms for the adopted animal(s) that may include: First Right of Refusal, Right to Veterinary Inspection and Required Updates and Foster period. Horses that need significant training will require the involvement of the selected trainer in delivering updates as specified in your Care Contract. *
By signing my name below, I declare that all of the information I have submitted to be the truth, to the best of my knowledge, and if found to be fraudulent, that I will be denied approval of adoption and I could be held liable for any damages incurred by RH Farm Horse Rescue.
