Swift River School Initial Return-to-School Survey
October 22, 2020

SRS seeks information as we prepare for our potential hybrid return to school. We are presently creating a Hybrid Protocol Handbook to detail some of the extensive health and safety plans associated with this phase. THIS HANDBOOK IS IN DRAFT FORM AND IS NOT YET COMPLETE. When the handbook is ready, it will provide an overview of the careful and thorough plans we will implement to promote a safe and healthy environment at SRS. We look forward to sharing it soon.

After the School Committee meets on November 5th to vote on the best plan moving forward, we will conduct an additional survey to finalize your family's requests. We will also host an information night in early November to provide insights into our health and safety protocols, classroom set-up, and learning models, and to offer a Q&A.

The following questions pertain to how your family feels about the possibility of sending your student(s) back to school. The proposed hybrid model would entail in-person learning from 8:30-11:30 AM (possibly using an alternating cohort model, depending on the number of students opting to return); all students would still participate in remote learning from 1:00-3:10 PM until the new year (and eat lunch at home).

Please enter the name of the guardian filling out this survey (your name): *
Please list your student(s) and their grade level(s): *
If SRS transitions to hybrid learning will your student(s) attend the in-school portion (most likely from 8:30-11:30 AM)? *
If you plan for your student(s) to attend school, will they need bus transportation? *
SRS is working hard to meet the needs of students and families during this unprecedented time, and we appreciate all the support from our families and community members. Please include any other questions or thoughts you wish to share below:
