Attendance has an impact on a student's success in school. If a student must miss school, it is his/her responsibility to obtain any missed assignments and make up all work, and to especially initiate if they have missed a test or quiz.

Students are granted make up work days equal to the time of the absence. In order for a student's absence to be excused, this form must be completed a WEEK PRIOR to the absence. Failure to do so may result in the absence being unexcused.

NOTE: UConn, Syracuse, Seton Hall students: You MUST check-in with your teacher to make sure you are not missing any major assessments (speech, test, lab, quiz, etc.) before you make you schedule your absences. Missing any of these assessments will lead to a 0 and possibly loss of credit for the class.

FAQ

Q: Does this replace calling the Attendance Line?
A: The form is not intended for use for partial days (early dismissal or late arrivals), it is for pre-arranged absences for a full day or more. If it's submitted, you do NOT need to call the Attendance Line (similar to when the paper document was turned in).

Q: Can I use it if I'm just going to miss one period for a doctor appointment?
No. If you have to leave early, you must still have your parent sign you at the security desk.

Q: Do I need to fill this out for School Related events (Retreats, Winterim)?
No.
