I give my student permission to attend Disciple Now 2021 at Sherwood Baptist Church.  By signing this form, I agree to hold harmless Sherwood Baptist Church and the adult sponsors from any and all liability, claims, or demands for personal injury, as well as damage and expenses of any nature that occur while my student is participating in this event.  I give permission for my student to receive medical attention, if required. (Parents will be contacted in case of an emergency.) *
