Journey Home Animal Rescue - Small Animal Adoption Application
Greetings! Thank you for your interest in adopting through Journey Home Animal Rescue.  

The adoption application has many questions that will help us match owners to the right pet.  Don't worry about having the perfect answers, just answer truthfully.  We ask some questions that a new pet owner might not have considered and may find helpful to think about prior to bringing a pet home and to help find a lifelong match.

The adoption application is the first step in meeting the pet you are interested in adopting.  Please fill in all the fields as the application can not be submitted with blank fields. We do background checks and call references.  WE REQUIRE ALL ANIMALS IN THE HOME BE SPAYED OR NEUTERED AND UP TO DATE ON ANNUAL CARE.

Your application is a general application to adopt through the organization. Once approved to adopt, you are approved overall. Please list the pets in the order you are MOST interested in or write "please recommend a match". Please note this pet may no longer be available once your application is approved due to prior approved applicants. We do our best to match applicants with well suited pets and also to update the website as frequently as possible. *

Is anyone living in the home allergic to animals or grass/hay?  (If yes, please explain how you will handle this.) *
Are all people in the home in favor of adopting an animal? *
List the pets currently in your home and pets within the last 2 years.  (Please include: name, age, sex, spay/neuter status, and how long you have had them). *
Have you ever owned a small animal before (not including a dog or cat)? *
If you've had small animals before, what happened to them? (e.g., passed away, re-homed, etc.). Please explain the circumstances. *
If you currently have any small animals, do you intend on introducing them to your small animal? *
The introductory process between small animals can sometimes be long, difficult, and requires dedicated time/effort. Have you researched how to introduce small animals? Please describe what your plan is. *
Will you keep your new small animal if you are unable to bond them? If no, please explain. *
Do all of your pets live inside your home?  (If no, please explain) *
Are your pets up to date with annual exams and vaccinations? *
Are all of your pets spayed or neutered?  (If no, please explain why not.) *
We require that small animals be housed indoors and in appropriate sized pens/cages. What sized pen/cage do you plan to use for your small animal and how many animals will be in this pen/cage? Where in your home do you plan to house your small animal? For example, in a spare bedroom, finished basement, etc. *
How many hours of playtime/socialization outside of the animals pen/cage will you provide daily? How do you plan to do this? (free range, leashed, exercise ball, etc.) *
How do your pets react to other animals? *
Hay, pellets, fresh greens, litter, and toys may cost $30-$60/month. Your small animal should also have an annual wellness check with an experienced small animal vet. Emergency visits, unforeseen illnesses, or injury visits can easily exceed $500. Are you willing to accept this financial commitment for your small animal? *
Veterinary Reference - Provide the clinic name and phone number of the clinic(s) of any pets you have owned in the past 2 years.  Also provide the name the records are under. (Please call your vet and give permission for Journey Home Animal Rescue to access your records.) *
Have you ever given away, sold, or surrendered a pet? Please explain. *
Explain where the small animal will be kept while you are away from the house and when you are on a trip. *
Do you acknowledge there can be disagreements and will you strive to ease the transition and provide care and guidance as needed?  (There can be fear and anxiety during the transition time- which can be anywhere from days, to weeks, to months- until the animal is comfortable in your home.) *
Do you plan to let your small animal outdoors? If so, what are your plans to keep the pet safe during this time? Harness? Cage? Exercise pen? *
What would be your reasons you would surrender your pet? Have you needed to surrender a pet in the past? If so, please explain.  (We require all pets to come back to Journey Home Animal Rescue if you cannot keep them in the future.  We expect our adopters to make lifelong commitments when pursuing an adoption.) *
I understand that Journey Home does NOT support or advocate for their small animals to serve as Emotional Support Animals (ESAs). We will not provide any documentation/certification for their rabbits to serve in this role. *
I acknowledge Journey Home Animal Rescue cannot guarantee behaviors when pets are introduced to new environments, pets, children, etc. *
Are you currently or have you ever been a foster/adopter/volunteer for Journey Home Animal Rescue? *
Journey Home Animal Rescue does their very best to keep the website and social media as up to date as possible. I understand that the pet I am interested in may not be available and Journey Home will do the best to provide recommendations or place me on the waiting list. *
I understand in order to keep my place in line for the pet I am interested in, I must reply within 48 hours of each e-mail communication or Journey Home will move to the next application in line. *
I understand that Journey Home Animal Rescue adoption fees are NON-REFUNDABLE once an adoption is finalized. This includes situations where the pet is surrendered back to Journey Home Animal Rescue. If the adopted rabbit is not a good fit for your home, we will exchange it within 60 days for a more suitable rabbit. *
