Matching Funds Notification Form
If you are requesting matching funds from an employer, please fill out this form so CF can verify the donation, track the matching check, and deposit the funds in the correct booster club account.

Please use the full name of our organization when requesting a match from your employer: Centurion Foundation of University City High School, Centurion Alumni Foundation of University City High School, or our federal tax ID number which is 33-0073915.
