The government has declared a State of Public Health Emergency (Proclamation No. 922) on 8 March 2020, which will take effect until withdrawn or lifted. On 12 March 2020, the Department of Health issued Alert Level 4 Code Red sub-level 2.  

As the health and overall welfare of the UP Mindanao community is of primary concern of the university, we are asking you to complete this COVID-19 case tracking survey to help us monitor the current health condition of UP Mindanao employees. Kindly answer this form as accurately as possible. The current instrument is a modified version of the Student Monitoring Survey originally used by the Office of Student Affairs. In compliance with the stipulations of the Data Privacy Act of 2012, the data collected in this survey shall be used by the university for crafting strategies to address the COVID-19 pandemic and for reporting to the UP System and other relevant agencies. Care shall be exercised to ensure that personally identifiable information shall not be shared and only approved personnel such as members of the Human Resources and Development Office can access the data.
