Please use this form to file a comment or formal complaint with the Office of the Ombudsman at the Flat Hat. This form is intended for comments and complaints of coverage, journalistic malpractice, ethics, factual inaccuracies, and personnel.

No anonymous submissions will be accepted. Your submission can be kept anonymous from the rest of the Flat Hat. The author or reporter involved may be made aware of the content of the complaint, but all anonymity still applies.

Not all cases will be investigated and those that do may result in no action.
