Minor Assent Form: Student engagement and program evaluation for Zero Robotics Education and Outreach Program
1. My name is Professor Danielle Wood.

2. We are asking you to take part in a research study because we are trying to learn more
about what students learn when participating in Zero Robotics, our educational
program that teaches students to code robots on the International Space Station. We
want to see what you think about your career, your academic interests and what you
learned before and after the program.

3. If you agree to be in this study we will ask you to speak to an interviewer and respond
to some questions in writing for about 30 minutes.

4. We do not expect that any risks will occur based on your participation in this study.

5. We expect that participating in this study will benefit you by allowing you to reflect
on what you want to learn from Zero Robotics and it will help our team at Zero
Robotics to improve the program for future students.

6. Please talk this over with your parents before you decide whether or not to
participate. We will also ask your parents to give their permission for you to take part
in this study. But even if your parents say “yes” you can still decide not to do this.

7. If you don’t want to be in this study, you don’t have to participate. Remember, being
in this study is up to you and no one will be upset if you don’t want to participate or
even if you change your mind later and want to stop.

8. You can ask any questions that you have about the study now. If you have a question
later that you didn’t think of now, you can email me at drwood@mit.edu or ask me
next time we meet. You can also call the Chairman of the Committee on the Use of
Humans as Experimental Subjects at M.I.T. at 1-617-253 6787 if you feel you have
been treated unfairly.

9. Signing your name at the bottom means that you agree to be in this study. You and
your parents will be given a copy of this form after you have signed it.

Student Survey Introduction
Hi There Kids!

Welcome to Zero Robotics.  We are so excited that you have decided to write code for robotics satellites on the International Space Station this summer.  While this challenge might be new to you - we know that you can do it!

We are asking you to take this survey at the beginning and at the end of your Zero Robotics program.  We want to see how much you (and your teachers) learned over the summer.  We DO NOT expect you to know all of the answers to the questions we are about to ask so please do your best and move on to the next question.  (Hopefully, you will know more answers at the end of the summer though!)

This is a KIDs ONLY Survey. It will take you approximately 15 minutes to answer these questions. Your participation is completely voluntary and your responses are strictly confidential.  If you feel uncomfortable answering any questions, you can withdraw from the survey at any point.

Good Luck!
